I understand that group fitness classes. and use of the equipment at RAM Fitness, LLC, poses a risk of injury and bodily harm and in extreme circumstances, could result in paralysis or even death.

By signing this form, I am agreeing that I am physically capable of attending and participating
in group fitness classes or using the exercise equipment at RAM Fitness, LLC.

In being permitted to participate in group fitness classes and use the RAM Fitness facility, I hereby release RAM Fitness. its owners, operators, and group fitness instructors of any liability for injury. death, or property loss suffered by me resulting from any ordinary negligence from the parties (RAM Fitness and those associated with RAM Fitness).

I acknowledge and appreciate the risks involved in participating in group fitness classes or using exercise machinery and release RAM Fitness of any liability associated with my participation. I waive my right (and the rights of my family, my heirs, and my assigns) to bold RAM Fitness accountably should I be injured in any way or suffer loss of life.
